§ 14-278. Contents of application.

An applicant for a license shall submit to the city clerk the following information:

(1)

Name and address of the massage parlor;

(2)

Full name and present address of applicant;

(3)

Three previous addresses of applicant and dates of residence at each;

(4)

Written proof that the applicant is at least 21 years old;

(5)

Two recent portrait photographs of the applicant at least two inches square taken within six months of application;

(6)

Detailed and complete business, occupation or employment history of the applicant for the ten-year period preceding the application;

(7)

Business license history of the applicant, including whether applicant has previously had a license revoked or suspended in this or another state, the reason therefor, and business activity or occupation subsequent to suspension or revocation;

(8)

All criminal convictions, except traffic offenses, and the reasons therefor;

(9)

Further identification and information if requested by the police chief to discover the truth of matters specified in the application;

(10)

The site plan and evidence that the location of the proposed massage parlor is within the proper zoning district and complies with the city's zoning requirements;

(11)

Where the applicant is a corporation, association, partnership, joint venture or private club, the information required in subsection (10) of this section shall be furnished for each officer of the corporation, each member of the board of directors of the corporation and each holder of at least ten percent of the corporate stock of any class; each member of the association, partnership, joint venture or private club;

(12)

The distance from other massage parlors in the cities of Federal Heights, Westminster and Thornton and the distance from any public or private school in the city; and

(13)

The anticipated number of masseuses and employees who shall work at the massage parlor.
